Modicon Quantum/Word Mapping

I'm working on a Quantum PLC that has some ladder moving transmitter values to a descriptor (e.g., Burner2_Core_Slv_Temp set to TT120469_Scaled x 10). I'm bringing this data over serially to a Honeywell TDC3000/HPM which uses flag and numeric arrays talking over MODBUS. Trying to connect the dots between say, a numeric address in the HPM and the descriptor in the PLC. Somewhere, there has to be some sort of data table mapping setting this descriptor to a MODBUS address. Can someone explain how this is done and how I can extract this data table?
